Health care workers with needlestick exposures to patients' blood often request a test of the patient for evidence of infection with human immunodeficiency virus. If the patient refuses the test, a conflict develops between the interests of the health care worker and those of the patient. Traditional approaches to this dilemma attempt to balance the rights or utilities of abstract patients and health care workers. While these approaches have the advantage of offering clear guidelines in advance of conflict, the interests (...) of the actual participants may differ from those used to create the guidelines. In nonmedical settings, conflicts are often resolved efficiently through negotiation and monetary exchanges. Although negotiated monetary settlements between health care workers and patients may be an impractical way to resolve medical conflicts, models developed from these perspectives provide insights into the individual interests of physicians and patients. Changing existing rules about medical record documentation, or increasing the penalties for the misuse of medical information, may satisfy the interests on both sides of the conflict and so represent integrative bargaining solutions. Even so, as the relationship between health care workers and their patients evolves, more explicit strategies for negotiation may become a reasonable solution to the problem of conflict. (shrink)

An analytic formula for special stretched Uq(3) Wigner coefficients, $$\left\langle {(\lambda _1 \mu _1 ) H_1 , (\lambda _2 \mu _2 ) \varepsilon _2 \Lambda _2 m_2 \left| { (\lambda _3 \mu _3 ) H_3 } \right.} \right\rangle _{\max }^q $$ is derived using (...) a projection operator method.(9–10) In this expression Hi denotes the highest weight vector of the (λiμi) irrep; the subscript “max” means coefficients corresponding to a unit tensor operator with a maximal characteristic null space; and q is the usual quantum label so the standard U(3) Wigner coefficient, which is required in the DA code, can be obtained in the q→1 limit of the theory. To illustrate the theory, some Uq(3) Wigner coefficients for the tensor product (22)×(22) are calculated. A geometric interpretation of gravitation is given using general relativity. The law of gravitation is taken in the formR 44=0, whereR 44is the component of the contracted Riemann-Christoffel (Ricci) tensor representing the curvature of time. The remaining curvature components of the contracted Riemann-Christoffel tensor may or may not vanish. All that is required in addition toR 44=0 is that the Gaussian curvatureR be nowhere infinite. The conditionR 44=0 yields a nonlinear wave equation. One of the static degenerate solutions represents the (...) gravitational field surrounding a static gravitational point singularity. It is found that for this solution, the three famous predictions of general relativity are obtained in the weak-field approximation. To that effect, a typology distinguishing a « denominational » poetry, a « confessing » one, and a third, indirect type, will be tested, according to the verbal influences that are to be found, or to the language innovations, or to the interaction of the references to the spiritual experience. It should eventually be obvious that the boundary line of the poetical creativity does not separate the first two types from the third one, as much as it runs between the first one and the two others, when the latter enable various interpretation. The concept of civil society has once again emerged as a viable mechanism for developing and sustaining deliberative democracy. However, an essential component of many strategies to sustain civil society appears lacking, especially when we see the growing cynicism and apathy among citizens. What is missing is a strategy for training or encouraging citizens to participate more fully in civil society. The skills of advocacy can, at least in part, help renew civic activism. While my own philosophical views are largely in keeping with my mentor, Dallas Willard, nevertheless, I find his conception of the human person puzzling, hard to specify precisely, and prima facie contradictory in a few places. Dallas's central goal in formulating his anthropology was to develop a model that shed light on, allowed for deeper insight into, and fostered interest in spiritual formation, especially the role of the body in spiritual maturation. I share this goal, and agree with most of (...) his model. But in what follows, I will make more precise what his views were, try to clear up what, prima facie, seem to be contradictions in his theory, and, finally, recommend an alternative that captures the central concerns Dallas had for his own position. Thus, I will lay out a few general points of Dallas's ontology, provide a description of his philosophical/theological anthropology, along with two problems that seem to be present, and offer a slightly adjusted alternative to his position that accomplishes his main goal in a way with which I believe he would be satisfied. Objectives: To determine the effects of risk and payment on subjects’ willingness to participate, and to examine how payment influences subjects’ potential behaviours and risk evaluations.Methods: A 3 × 3 , between subjects, completely randomised factorial design was used. Students enrolled at one of five US pharmacy schools read a recruitment notice and informed consent form for a hypothetical study, and completed a questionnaire. Risk level was manipulated using recruitment notices and informed consent documents from hypothetical biomedical research projects. Payment (...) levels were determined using the payment models evaluated by Dickert and Grady as a guide. Five dependent variables were assessed in the questionnaire: willingness to participate, willingness to participate with no payment, propensity to neglect to tell about restricted activities, propensity to neglect to tell about negative effects, and risk rating.Results: Monetary payment had positive effects on respondents’ willingness to participate in research, regardless of the level of risk. However, higher monetary payments did not appear to blind respondents to the risks of a study. Payment had some influence on respondents’ potential behaviours regarding concealing information about restricted activities. However, payment did not appear to have a significant effect on respondents’ propensity to neglect to tell researchers about negative effects.Conclusions: Monetary payments appear to do what they are intended to do: make subjects more willing to participate in research. Concerns about payments blinding subjects to risks could not be substantiated in the present study. However, the findings do raise other concerns—notably the potential for payments to diminish the integrity of a study’s findings. Future research is critical to make sound decisions about the payment of research subjects. (shrink)

Context The conduct of medical research led by Northern countries in developing countries raises ethical questions. The assessment of research protocols has to be twofold, with a first reading in the country of origin and a second one in the country where the research takes place. This reading should benefit from an independent local ethical review of protocols. Consequently, ethics committees for medical research are evolving in Africa. Objective To investigate the process of establishing ethics committees and their independence. Method (...) Descriptive study of 25 African countries and two North American countries. Data were recorded by questionnaire and interviews. Two visits of ethics committee meetings were conducted on the ground: over a period of 3 months in Kigali (Rwanda) and 2 months in Washington DC (USA). Results 22 countries participated in this study, 20 from Africa and two from North America. The response rate was 80%. 75% of local African committees developed into national ethics committees. During the last 5 years, these national committees have grown on a structural level. The circumstances of creation and the general context of underdevelopment remain the major challenges in Africa. Their independence could not be ensured without continuous training and efficient funding mechanisms. Institutional ethics committees are well established in USA and in Canada, whereas ethics committees in North America are weakened by the institutional affiliation of their members. Conclusion The process of establishing ethics committees could affect their functioning and compromise their independence in some African countries and in North America. (shrink)
